②The deadline for applying from this form is 10 days before the date of entry into Japan.
If you apply from this form by the deadline, you can use internet service from the date of entry.
このフォームでの受付期限は、入国日の10日前です。
期限内にこのフォームから申し込みされた方は、入国日からインターネットが利用可能です。
③If you apply using this form and cancel your application after entering Japan, you will also need to submit your credit card information. If you cancel, you will be required to pay the internet usage fee and equipment installation fee of 19,800 yen from the date of entry to the date of request in one lump sum.
このフォームから申込をし、入国後申込キャンセルする場合もクレジットカード情報をご提出いただく必要がございます。キャンセルする場合は、入国日から申し出があった日までのインターネットご利用料金と機器設置費用19,800円を一括でお支払いいただきます。
④Person who could not apply by the deadline, please submit the application form to Toyota Smile Life Office after entering Japan.
After receiving it, we will contact you to arrange the schedule for the Internet installation.
期限内に申込ができなかった方は、入国後にトヨタすまいるライフ事務所へ申込書を提出してください。
申込書受領後、インターネット設置工事の日程調整のため、ご連絡させていただきます。
